Statutory Declaration
The CCDC produces two separate Statutory Declaration forms. Using the wrong one is a common and avoidable error. The distinction is simple β it depends entirely on your role in the contract.

CCDC 9A
Prime Contractor
You hold a direct contract with the Owner and are applying for a progress payment or holdback release.

CCDC 9B
Subcontractor
You hold a contract with the Prime Contractor (not the Owner) and are applying for a progress payment or holdback release.

Both forms are free at ccdc.org and available through your CASK office. The forms can be completed digitally in Adobe Acrobat before printing. If you only have Adobe Reader (free), print first and complete by hand β Reader does not save form entries.
A Stat Dec is required as a condition of payment β but it doesn’t always need to be submitted at the exact moment you send your invoice. The timing depends on the payment type.
A commissioner will not sign a form that is incomplete, blank in any section, or contains errors. This is a legal document β it must be accurate and complete in every respect before you attend a commissioner.
Before attending a commissioner, a CCDC copyright sticker must be physically affixed to the designated box in the lower right corner of the form. This is a legal requirement β not an optional step.

Commissioning a Stat Dec has strict rules. Getting this step wrong β even with a perfectly completed form β will invalidate the document entirely.
The declaration must be signed by an authorised signing officer of the company β the President, Vice-President, Secretary, or another officer specifically authorised by board resolution. This cannot be delegated to a regular employee.

All CASK offices have qualified Commissioners of Oaths and Notary Public on staff β at no cost to members. No appointment needed at most locations.
Run through this list every time before attending a commissioner. A single missed item means a wasted trip.
